AGREEMENT OF RELEASE AND WAIVER OF LIABILITY FORM I, hereby agree to the following: I am participating in the Power Yoga Vinyasa Class, Studio B class, or Workshop , offered by Orlando Power Yoga, LLC. during which I will receive information and instruction about yoga and health. I recognize that yoga may require some physical exertion, which may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in the Power Yoga class or Workshop. I represent and warrant that I am physically fit and I have no medical condition which I have read the above would prevent my full participation in the Power Yoga Class/Workshop. Otherwise, I am fully aware of any medical condition I have and have received approval from my doctor to participate in the Hot Power Yoga Class/Workshop. Orlando Power Yoga is not liable for any injuries or illnesses including death related to my health condition. In consideration of being permitted to participate in the Power Yoga Class or Workshop, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in the program. In further consideration of being permitted to participate in the Power Yoga Class/Workshop, I knowingly, voluntarily and expressly waive any claim I may have against Orlando Power Yoga, LLC., it’s owners, and class/workshop Sponsor, for any injury or damages that I may sustain as a result of participating in the program. I, my heirs or legal representatives, forever release, waive, discharge and covenant negligence or other acts. I acknowledge I am participating in a yoga class, which is a group activity, during the Covid-19 Pandemic. I acknowledge that I might be exposed to said virus, and accept all consequences thereof, including sickness, debilitation, and death. I specifically release Orlando Power Yoga and Studio B from any damage associated with such expose, and consent to such exposure with full knowledge of any potential ramifications. I further agree that all packages have expiration dates and that there are no extensions on such packages. I have read the above release and waiver of liability and fully understand its contents as well as the Refund/Cancellation and Make-Class Policies. I voluntarily agree to the terms and conditions stated above.
